Based on the readings assigned this week, develop a care coordination plan to be executed by the case manager for the first four weeks post discharge. Consider the needs outlined in the case study, as well as any additional needs that the case manager should consider. Be detailed in your description of the care coordination activities of the case manager. Support your positions and thinking with sound reasoning, substantive evidence, and citations of sources when applicable.
